Brief description of role or responsibilities​
The Pharmacists Clinic team provides comprehensive medication management consultations as well as minor ailment assessments and prescribing within a primary care setting. They help patients get the most out of their medications and empower them to be active decision-makers in their health journeys. The clinic team supports patients and their healthcare teams in implementing, monitoring, and adjusting medication therapy. The pharmacists are also involved in pharmacy education and practice-based research.
Describe how the individual or team made a positive contribution to patient care or their pharmacy department:
The Pharmacists Clinic team continues to pioneer innovative approaches to expanding access to pharmacist expertise. As collaborators in the Real Time Virtual Support (RTVS) Quick Reply pathway, the team provides on-call virtual support to rural and remote healthcare providers across British Columbia, addressing medication-related questions and helping to bridge the gap in health care access for underserved communities. The clinic also serves as a study site for the CIHR-funded TEC4Home-BP study led by Dr. Karen Tran, which evaluates the use of home blood pressure telemonitoring combined with pharmacist-led medication adjustments for patients with hypertension identified in emergency departments. This research may transform how we manage chronic diseases in the outpatient setting. Additionally, team members actively contribute to pharmacy education through their teaching roles within the UBC E2P PharmD Integrated Activities team, ensuring the next generation of pharmacists is trained in patient-centered, evidence-based care. The team will also work in close collaboration with Gateway Team-Based Care Teaching Clinic, a new purpose-built interdisciplinary clinic on the UBC Vancouver campus. Through these initiatives, the Pharmacists Clinic demonstrates how academic pharmacy practice can simultaneously advance patient care, health system innovation and professional education.
