Brief description of role or responsibilities​

Our pharmacy technician plays a vital role in supporting the anticoagulation clinic by coordinating access to low molecular weight heparin (LMWH) for patients who require bridging or new anticoagulation therapy. They help ensure the correct medication strengths are available when needed and assist in sourcing these medications from other pharmacy sites when supplies are limited. By managing these logistics efficiently, they help the clinical team focus on patient care while ensuring medications are accessible in a timely manner.

Describe how the individual or team made a positive contribution to patient care or their pharmacy department:

Our pharmacy technician plays a key role in ensuring patients receive timely anticoagulation therapy. When specific low molecular weight heparin strengths are difficult to find, they take the initiative to contact other pharmacy sites, coordinate transfers, and secure the medication as quickly as possible. Their proactive approach helps prevent delays in starting or continuing treatment, which is especially important for patients requiring perioperative bridging or urgent anticoagulation. Through their dedication and collaboration with the pharmacy team, they help ensure our patients have reliable access to the medications they need and support safe, seamless care.
