Claire MacLeod

Pediatric Clinical Pharmacy Specialist

Profile submitted by: Jay Jeong and Gurneet Rana on behalf of SMH Neo & Peds Pharmacy

Brief description of role or responsibilities

Claire is a clinical pharmacy specialist in Pediatrics at Surrey Memorial Hospital. Every day, she plays an important role in optimizing medication care plans for her patients. Through her work with the Fraser Health Maternal Infant Child and Youth (MICY) Medication Quality and Safety Committee, PDTM, and Meditech Expanse, Claire ensures that medications are used effectively and safely in an especially vulnerable population. In between her clinical and committee work, she readily precepts countless pharmacy students/residents. In addition, Claire maintains a leadership and mentorship role for the Neo & Peds Pharmacy team.

Describe how the individual or team made a positive contribution to patient care or their pharmacy department:

It is truly inspiring to see Claire work to create a difference for her patients and the pharmacy profession. I was lucky enough to have Claire as a preceptor during my time as a resident. Claire is knowledgeable, supportive, non-judgemental, flexible, and dedicated. She encompasses all the qualities that a great mentor should have and leads by example. It is not uncommon to find Claire going above and beyond to ensure her patients are receiving the best care possible. Through this, she demonstrates to everyone the value of having a clinical pharmacist as part of the interdisciplinary team.

Claire’s presence on the team has allowed me and other pharmacists feel welcomed and appreciated by the pediatricians, nurses, and other allied health professionals. Her nurturing teaching style and collaborative personality inspire us to become better versions of ourselves. Thank you!
