Brief description of role or responsibilities

Jane is a Clinical Pharmacy Specialist working in the Emergency Department at Richmond Hospital. She looks after patients requiring urgent care. A few of her responsibilities include: recognizing and reporting medication related adverse reactions, participating in simulations, and aiding physicians in making medication-related decisions.  

Describe how the individual or team made a positive contribution to patient care or their pharmacy department:

Jane makes a positive contribution to patient care by leading research projects aimed at improving patient outcomes. She consistently goes out of her way to address medication-related concerns leading to Emergency Department visits. One example comes to mind regarding the improper use of maintenance inhalers leading to COPD and asthma exacerbations. 

She recognized the issue and worked with other healthcare providers to create patient handouts and learning modules to better inform patients on the importance of their inhalers, and what to do if they experience signs and symptoms of an exacerbation. 

Although many pharmacists participate in research, Jane stands out because every time she talks about one of her projects, her passion for improving patient care shines through.
