Brief description of role or responsibilities
The Pharmacists Clinic team provides comprehensive medication management consultations as well as minor ailment assessments and prescribing within a primary care setting. They help patients get the most out of medications for chronic conditions and support them in being decision-makers in their health journeys. The clinic team then supports patients and their healthcare teams in implementing, monitoring, and adjusting medication changes. The pharmacists are also involved in health professional education, health system development and research.
Describe how the individual or team made a positive contribution to patient care or their pharmacy department:
In addition to providing direct patient care, the Pharmacists Clinic team works as a living laboratory to support health system development and health professional and student education. For example, they have supported the implementation of pharmacists in primary care networks across the province, were the first community pharmacy in BC with access to CareConnect, helped launch the COVID antiviral support line for clinicians, and developed an opioid stewardship referral program between hospital and primary care settings. Recently, they worked with the First Nations Health Authority to develop two courses on building cultural safety and humility for First Nations clients into pharmacy practice to help pharmacy team members learn how to address systemic anti-Indigenous racism and barriers to healthcare access.
