Brief description of role or responsibilities​

The qathet Primary Care Clinical Pharmacist works as a team with primary care practitioners, nurses, and social workers to increase capacity, improve patient outcomes, reduce hospitalization, and streamline care. They help address gaps in primary care for patients with complex health conditions. In qathet, the PCCP adds 40-60 additional appointments per month and has been instrumental in supporting physicians,
nurses and social workers to with timely management of medication interactions, medication availability to inform prescribing, best possible medication reconciliation, post-discharge support, and financial barriers to prescribing.

Describe how the individual or team made a positive contribution to patient care or their pharmacy department:

Lindsay is making a difference in our community as the new PCN pharmacist. Her bright and cheery disposition along with her exceptional knowledge of pharmacy helps the clients receive the absolute best care possible. Lindsay is efficient and knowledgeable in both the clinical aspects as well as the human aspects of her discipline and she brings her A game to any task assigned to her. Lindsay’s efficiency helps lighten the load for the clinicians and providers in the PCN. She is always willing to take on a challenging case and her expertise is valued by the team as decisions are made.
