Brief description of role or responsibilities​

Positive Health Services in a multi-disciplinary team located at the Jim Pattison Outpatient Care and Surgery Centre in Surrey. They manage HIV and hepatitis C care for 700 patients in Fraser Health. Their success relies on seamless collaboration among a team of nurses, nurse practitioners, physicians, pharmacists, social workers, clerks, and peers. Regular communication and shared decision-making ensure good patient care, which includes HIV and primary care, hepatitis C treatment, sexual health, and medication management. The team also provides support for psycho-social needs such as barriers to care, stigma, housing, addictions, nutrition, peer navigation, and mental health.

Kudos to the exceptional team at the Positive Health Clinic, where dedicated nurses, nurse practitioners, ID physicians, pharmacists and allied health workers provide comprehensive care to patients living with HIV and hepatitis C. Their unwavering commitment to patient well-being makes a profound difference in the lives of those they serve. Each professional plays a vital role to ensure the highest standard of care. The team creates a patient-centered environment that serves people living with HIV and hepatitis C but also acts as a vital resource for other healthcare providers in Fraser Health, sharing knowledge and best practices to enhance care in the boarder community.
