Brief description of role or responsibilities​

Jin is a clinical pharmacist at Ridge Meadows Hospital with an interest in internal medicine.

Describe how the individual or team made a positive contribution to patient care or their pharmacy department:

Jin enthusiastically took on a three-month project to explore the pharmacists’ role in our Medical Transition Unit, which houses patients who are awaiting admission to the hospitalist program. Here they can wait several days for a full admission work up, including medication reconciliation, with their care being covered by a rotating list of physicians (often ED docs). Our department identified this population as being high risk for lack of continuity of care and Jin jumped in to run a QI project tracking pharmacist interventions to support patient safety and quality of care. He was able to summarize the data and present his findings to site leadership.

This project highlighted the importance of the pharmacist on the multidisciplinary team with one physician writing:

The clinical pharmacist is an essential component to the MTU team especially since the majority of the MTU doctors don’t provide inpatient care normally and so the pharmacist regularly helps us avoid medication errors or inappropriate treatments. It is also a unit where the doctor changes every day and so having a consistent person reviewing charts allows for advocation for consistency in care for patients.
