Brief description of role or responsibilities
Donna is one of the Richmond Primary Care Network pharmacists and provides support directly to the providers through co-location within Richmond or within the team, as well as supporting the education and development of students and fulsome integration of team-based care work at the Primary Care Networks.
Describe how the individual or team made a positive contribution to patient care or their pharmacy department:
Donna puts the clients at ease with her genuine and sincere nature. She cares so much about the clients and advocates for them and puts in the extra work to ensure they have the best outcomes possible. Donna has great ideas that truly represent shared care models and team-based care models, providing frameworks that identify populations that would be better served by adjusting our service delivery model. This shows how much she understands and advocates for the clients in Richmond, and we are really lucky to have her as part of our team!
