Brief description of role or responsibilities
Angela is the sole clinical pharmacist at Timber Creek Tertiary Care Facility. She wears a number of different hats as she helps patients in the psychiatric intensive care unit, assessment, treatment and rehabilitation unit and the tertiary intensive rehabilitation unit. Angela is one of the few team members that overseas the care of essentially every patient at Timber Creek with medication reconciliation, treatment recommendations, monitoring and discharge planning.
Describe how the individual or team made a positive contribution to patient care or their pharmacy department:
In the beginning of the rotation, managing psychotropic medications seemed quite complicated at first, but Angela was able to explain key concepts in a way that that made everything make sense. Angela is a very dedicated pharmacist who cares deeply for her patients. This is reflected in the way that she ensures that adequate information is given during the transfer of care process. She is deeply invested in the success of her patient’s treatment. As one of the few clinicians who deals with almost every single patient at Timber Creek, Angela is very dedicated to ensure the appropriate treatment for not just the patients’ psychiatric conditions but their medical conditions as well. Additionally, Angela’s passion for teaching is very clear as she makes ample time to talk about patient cases and share very valuable clinical pearls amid her extremely heavy workload. I especially enjoyed her stories regarding unique pharmacological approaches in managing patients at Timber Creek and her memory is second to none.
