Hospital at Home: Team-Based Acute Care Beyond the Hospital

About the Session:

This session introduces the Hospital at Home (HaH) model, exploring its core components and the essential role of pharmacy in managing medications safely outside traditional hospital settings. The presentation highlights how interdisciplinary collaboration and patient oriented research are driving improvements in care delivery and supporting the growth of HaH programs across Canada.

About the Speakers:

Brittney Mathers is a clinical pharmacist with Providence Health Care. She has been working with the HaH team since April 2024 and previously practiced in internal medicine and oncology. Brittney is passionate about patient-centered care, supporting safe medication use in the home setting, and advancing collaborative, team-based care.

Jeff Tian Seng worked as an RN since 2009, initially in the ED at Richmond Hospital and eventually as a PCC and shift coordinator. In 2016, he moved to Kelowna, working in the ED and as a PCC on the medical floor. Jeff transitioned to the HaH PCC role at Kelowna General Hospital in January 2026.

Pansy Shiu is a Pharmacy Technician who has been with Providence Health Care since 2007 and joined HaH team in June 2025. She enjoys being part of a team that supports patients recovering at home and finds the day-to-day variety of HaH especially rewarding.

Dr. Sean Spina is a clinical pharmacy leader and researcher at Island Health, focused on advancing innovative, patient-centered models of care through real-world evidence. As Regional Clinical Pharmacy Manager and Principal Investigator of the AT-HOME research program, he leads initiatives that evaluate new care models, technologies, and clinical workflows to improve patient outcomes and health system efficiency. He has played an important role in advancing Canada’s HaH model. Dr. Spina also serves as adjunct faculty at the University of British Columbia, where he mentors healthcare trainees in clinical pharmacy and research.

 

Primary Care Pharmacists in Action: Teaming Up for Better Care

Speakers: Emilie Lamoureux & Corey Jefferies

About the Session:

Drawing on their experience in the East Kootenay Primary Care Network and the Oceanside Primary Care Network, Emilie Lemoureux and Corey Jefferies will provide an overview of how pharmacists collaborate with social workers, registered nurses, physicians, nurse practitioners, dietitians, mental wellness clinicians, and respiratory therapists to support patients across the continuum of care. They will also demonstrate how pharmacists partner with other providers to deliver both group and individual education sessions, including diabetes management and COPD or asthma education, helping strengthen chronic disease support and enhance patient self management.

About the Speakers: 

Emilie Lamoureux is a clinical pharmacist and a certified diabetes educator. She is currently working with the East Kootenay Primary Care Network with Interior Health since February 2022. She holds a PharmD from the University of British Columbia. Emilie’s prior experience is from community pharmacy. Her current practice is focused on chronic disease management with a focus on diabetes care, and migraine management.

Corey Jefferies is a clinical pharmacist and Certified Diabetes Educator working with the Oceanside Primary Care Network on Vancouver Island. He holds a BScPharm and a post graduate PharmD from the University of Alberta and has been working in Primary Care since 2017. Corey worked with the St. Albert and Sturgeon Primary Care Network in St.Albert, AB before transitioning to his current role in April of 2021. Corey’s practice focuses on chronic disease management with a focus on diabetes care.

This panel will highlight AI initiatives underway within BC health authorities and explore what these developments could mean for the future of pharmacy. Panelists will discuss how AI may support clinical decision-making, workflow efficiency, and patient safety, while also considering implementation challenges, evolving roles, and key risks. Join us for a practical discussion on how pharmacy can help shape the future of AI in healthcare.

About the Speakers: 

France Mytropan is a health informatics leader spearheading complex health technology projects across acute and ambulatory care. With a background in clinical pharmacy and a master’s degree in health administration, France strives to bridge the gap between clinical requirements and technology, fostering the adoption and integration of digital solutions in healthcare. In her current role as the Director of Clinical Informatics at VCH, France collaborates closely with senior leadership and clinical operations to advance the use of technology and find innovative solutions to problems.

Josh Batterink is a graduate of the UBC Faculty of Pharmaceutical Sciences and the VCH-PHC Residency program. He worked as a clinical pharmacist at St Paul’s Hospital before moving into pharmacy informatics roles at PHC and FHA. He is currently a LMPS director for Pharmacy Informatics, Medication Safety, Quality and Parenteral Services.

Steven Du is a graduate of the UBC Faculty of Pharmaceutical Sciences and the LMPS Residency program. He has worked as a clinical pharmacist at the Vancouver General Hospital and in various pharmacy informatics roles implementing Cerner and Meditech Expanse. He is currently the Manager of Pharmacy Informatics and Automation for Lower Mainland Pharmacy Services.
